I think Accutronix makes one, and I'm sure Kuryaken makes ones too. Check Dennis Kirk and/or J&P Cycles though, they should have them there, even if you purchase them somewhere else (cheaper most likely!)
Larry5H: check out Baker custom. I have the extended toe and heal shifters and they work great. Saw them at Laughlin last year. My buddy bought them there and I ordered over the phone and had them in a few days. Install was simple - remove OEM and install new.
They add I think about 3 inches total to foot space. It works for me.

Try http://www.waltersworkshop.com/ -- I have the extended heel shifter on my SG. Works great, gives you about 2 more inches - made a big difference for me. Do a search of the forum - you'll find some more info on this topic.
These guys over at softbrake I've been really happy with... you purchase them seperate to custom the stretch length both forward and backward. You can get the extended brake lever as well. Anyway I highly recommend them.
