Default Brake pad fell out.

I have a 2011 Fat Boy Lo. My inboard rear brake pad fell out of the caliper and got jammed in there crooked. I removed the caliper and swung the pad back into place and bolted it back together. The piston side looked pretty worn out and the one that fell out looked like it had a lot of life left. What would cause this? I was using it at a MSF experienced rider course and it was close to the end of the day. Is this just from over use? Will a new set of pads take care of this or should I be looking at something else?

Been about a year since I had a brake pad in my hand but I would swear there are two pins (bolts) that go through the pad so it can't just slide out unless it was put in wrong. Or are you saying the friction material by itself came out? If so must be a bad rivet job on the pad. Either way it would be time for all new pads if it were me.

On the softail rear there is only one pin that holds it in place, it's on one end and on the other is a tab that sticks out. The tab end is the one that came out.

I installed new brake pads last night. I must not have put them back together correctly when I installed the new tire. I somehow made it 4000 miles before I had this issue. I'm blessed that this didn't happen while I was out in the middle of nowhere.
