...and throw in heavy boots and a heel/toe shifter with an obese rider stomping away. If the bike is set up correctly it take very little effort to change gears for a good rider.

This just happened to me, 1991 fxr as i was cruising along and lightly downshifted into 4th, it went to neutral. So, I went back to fifth and down shifted a little firmer and found 4th. I never had this happen once on my twin cam dyna in 38k miles. Kinda freaked me out at first but after reading the comments here I'm relieved.

Must Complete the "throw"... again... Push or Pull to the End not a quick short Jerk...
