Default No Clutch Cable Adjustment Left

I was riding the other weekend when after disengaging the clutch and shifting the lever did not travel all the way back. The bike still shifted fine and I found neutral easily when I came to the next stop light. I did a clutch adjustment today but the cable ran out of adjustment when I took up the slack in the cable. I was adjusting it per the manual to 3/16 - 1/8 inch. It's at around 1/4 inch now and the lever still does not return fully to the forward position. Does this mean my clutch or clutch cable needs to be replaced? The clutch doesn't slip. I've never had this happen before. I replaced the clutch on my Sportster after it had 42k miles on it out of abundance of caution (didn't want to experience spring plate failure).

It might help if you provide a little information as to what bike you have.
That said, I'm riding an '85 FXRS. ANYTIME I have an issue with clutch adjustment I remove the right side transmission cover and make sure everything in there is good.
That's my suggestion for any model.

It’s a 1990 Heritage with approximately 25k miles on it.

With the 1340s it's NOT unusual for the cable adjustment to be, or seem to be nearly maxed out, even with all new parts. First thing to do if you've not already done it, collapse the cable, adjust the clutch pressure plate no more than 1/2 turn off touching the push rod then adjust the free play with the cable.

The '87 up models right side release mechanism is different than earlier models and failure of the throwout bearing or anything on the right side is very rare.
