Never done that but I have pulled the 'ol jeans stuck on the peg while trying to put my foot down thing. Luckily I always solved the problem before it was too late.
I had it happen only once years ago, I stood back and mumbled to myself "dumb%$$", then figured out what I did wrong, didn't get my feet up soon enough.
This past week, my wife and I were riding 2 up on my 2013 Ultra Classic and as I took off I got my left boot caught under her passenger floorboard and laid it down on the left side. Luckily, it didn't put a scratch on the bike, but I'm now wearing a cast on my left foot.
I don't carry passengers, so the first thing I did with both my Road Glides when I got them home was get out some tools and remove the passenger floorboards.

And that was that!
Same here - I never ride two-up, I'm strictly solo. I go where I want - eat where I want, and eat when I am hungry - With the exception of the Toys-4-Tots run, I also never ride in large groups... Myself and a buddy or two and that's it - Period

Nope...trying to picture how the hell it could happen. Only thing I can come up with ...is duck walking or foot dragging. Sorry for your accident, but your misfortune is one of the many reasons not to do that.
Feet up the very same second you start moving, feet down right after you reach a complete stop..or at least right as you stop...not sooner. Can't do those...practice practice practice.
