Just developed similar problem in my neck/shoulder a month or so ago. Going to try the Khrome Werks 2+2 Narrow Sweepers. They're 2 in. higher, 2 in. back, and the same width as stock bars.
Back rest may move you a bit closer. I ordered some adjustable Helibars for my Limited. I will get them installed next week. Expensive, but you can adjust them to any position.
Same thing on the first few rides in Baja on the dirt bike. Felt like somebody was trying to stick a knife in my back between the shoulder blades. After a few more trips it went away.
My 19 year old employee on a new Wide Glide just told me he has the same thing when on a longer ride. I suggested he put up with it for a few more rides and then decide if anything needs changing. Our ride last Saturday he said it was much better.
I was experiencing the same pain,so after doing some researching here on the Forum I decided that the handlebars were the culprit.I bought a set of WO 575 and longer cables,had them installed but after an hour or so of riding the pain resurfaces.I like the way the new handlebars perform but I haven't solved my problem (pain),lately I'm having this pain even when not riding,will have to schedule a visit with an orthopedist.

I've had the same problem with just about every Harley I've owned, except an old FXR that had buckhorn bars. When I purchased the Fat Boy, I put on some apes that brought my hands to shoulder height. Can ride pain free forever now. I have some friends who have gone the Heli-Bar route and they like them. They look good on a full dresser & they are not so expensive when you consider you don't have to change any cables.

Your lucky I can only ride for around 45 minutes at a time before I get that kind of pain, I get off the bike and the pain disappears usually in seconds but I have to wait about 1/2 an hour before I can continue riding, I started having the problem some years ago with drag bars so I changed to Harleys mini apes which are better but not a lot, I'm seeing a chiropractor at the moment which is starting to make a difference, the X-rays they took showed that my spine looked like a snake had small unwanted bends in it from top to bottom, according to one of the guys I see when I asked him how many people have curves in their spines that arnt surposed to be there he laughed and said around 80%, I think a lot of people now are developing the dreaded iPhone neck.
I had the same pain on long trips with my stock SG seat and HD 12" apes. I bought a Sundowner and it moved me forward a little and gives more lower back support. Problem solved
