I don't think it's out dated but rather has more to do with where you live/ride. Here in So Cal I see it often but mostly in the canyons on the twisties where the LEO'S are tucked into a little corner waiting for (mostly) sport bikes to come zooming by dragging their knees around the corner.

I've seen the head tap a number of times around here. One day a bike coming toward me made a two fingered peace sign just above his forehead, that one I didn't know. A little further down the road there were a few deer on the side, so I assumed he was making an antler's ahead signal. I'm not sure if that is common or just that guy doing it.

It's still a thing here in the NW...learned it from my buddy's dad when I got my first GSXR750 at age 19...he was a Harley guy. Been doing it ever since.

Yes I’ve heard if it and do it all the time. If you’ve ever ridden in a really good, tight group that has their stuff together you learn the signs either pre ride or just by riding with them. I find it’s usually solo riders or those that are new and self taught that don’t know the road vernacular. I didn’t know it for years because I rode solo and never with an organized group.
