I have always been told that the heel shifter was added for the police bikes. They did not want their uniform boots scuffed.
I don't know if that's true or not, but that definitely was not the NYPD as the NYPD takes the heel shifter off all their bikes as soon as they get them.
As for me, I have two bikes, one with & one without. I always use it with the bike that has it. The toe shifter on that one is too low and it's hard to get my foot under it.
