I brought the flush mount axle for my rocker, from Puzzle a member of HDF,but I'm not understanding the installation instructions on how the axle is suppose to stay together. I know they work because there have been others on HDF who have them on their rocker's. one end of the axle has threads which screw on to the cap. the other end has a hex head with nothing to hold the cap on,thats my problem.
the left side flush mount end cap is round in diameter, the left side axle is a hex head (square) smaller than the round end cap,so I'm not seeing how the two can be secured together (left side).
do you have the heartland (hub) type or the flush mount, can't tell from pic.

You should be able to tap the splined Knurled cap (female end) in the lower left fork axle hole just to get it started. Then I took a bolt that had the same thread as the knurled cap and a washer to pull the cap the rest of the way in until it was flush. Don't forget to put Anti-Seize on the splines before pulling it in.
Once that's done all you have to do is put the axle through the wheel and spacers and torque it down. Then there is a flush cap that snaps in over the axle head on the right side.
