My fender hole solution...
What if you got an extra OEM screw that fit the threads.
Grinded it down a little flatter and either Crazy glued it to the underside of medallion or use JB Weld. You could use a black rubber washer to fill the gap where the screw head lifts the medallion off the fender.
I would set the screw in first so when you glue it, it is lined up the way you want it on the fender.

Actually, it's pretty easy to replace that anchor without removing the wheel or the fender (I had to do it once). Just run a coat hanger wire (or baling wire) down through the hole, and out the side of the fender. Then put the anchor over the end of the wire, and put a bend in it so it can't fall off. Then just pull the anchor into place with the wire, and put the snap ring on it.
The problem is that its not just an empty hole. It has that bit of hardware mounted in it with the black plastic snap ring. I'm sure that if I remove that snap ring, the screw hole hardware will drop through to the floor, and I can't reach far enough up under the fender, with the tire there, to put it back.
This is one of those little annoyances that will drive people crazy until they get the perfect solution. I would think a little chrome round head screw with a concave underside would be perfect. Snug right up to the fender with a rubber washer.

When i removed the stock seat and luggage rack on my Deluxe i ended up with 3 holes in the rear fender.Went on ebay and bought some chrome license plate screws for Harley's.A little locktite on them and a chrome washer on the one where the passenger seat was mounted made it looking clean.

..especially when I relocate my license plate. (I'll have 3 more holes to deal with).

If you could get your hands on a grinding wheel it would take about 5 minutes to grind. I would just thread a bunch of bolts on the threads and then grip it with a vise grip. That way you don't destroy the threads.
And definitely wear goggles incase that baby flies off!
If you can just go with it plain you could use that 1/4-20 screw and get a black plastic screw cap that fits over it at the hardware store or Home Depot. The caps are only like a dollar ea.
