Man that is really cool! I've never seen something like this. Jockey shift seems the way to go. There may be some options to shift on the bars as well, which would be optimal.

Thanks. Once I finished up with the work and adjustments, I cleaned and polished it up and it looked much nicer. It was a gift from his sister and I because of his condition. She took it to him on a trailer and I was told he was very emotional when he got it.

You could also go with an air shifter with buttons on the handle bar controls.

Look around for sites on disabled bikers, they are out there and probably have solutions for most things.
