Sounds like someone may have put some 7/8" bars on the bike along the way (I believe stock for yours are 1", but verify that).
A quick fix would be to loosen the bars, wrap some duct tape 2 revolutions around the bars where they are pinched for tightening, recenter the bars, tighten, and use a razor blade to cut off any tape sticking out. Best bet would be to measure the bars. IF they are 7/8", and your bike should have 1" bars, get some new 1" bars...
Get a new top plate and risers if the bars are the correct size. If they have been loose for a while these parts may be just out of round enough to cause your troubles.
I'm going to try a new top clamp (that means a new chrome speedo bracket too). She was wanting a "Live To Ride" clamp. I wonder if she's been loosening them up to get new parts?
Take a soda can... cut two 1 inch wide strips out of it... take the bar clamp off wrap the aluminium around the bar trim it to size and put the clamp back on torque to specs... works every time...
