I lowered my front end 1" to compensate for the 1.25" the larger wheel raises the front. (this is only a suggestion, I do not think you have to do this) The fender may need to be modifed slightly, I had to elongate the rear holes of the fender to get the proper clearance. I used a nut taped to the wheel to make sure I had .250" clearance.
I thought the handling improvements were dramatic on my bike, probably about 50% due to the Metzler tire design, and the rest due to the shorter sidewall of the tire / larger wheel diameter.
Go for it, you'll love the improvement, and the look
I just went 18-18 and after I did it wish I would have gone ahead with the 21-18 but I am very happy with the new look and dont have those damn spokes to clean any more
Definitely the last thing I'd modify on my FLHR. If I wanted a taller skinny tire to distribute less weight across the road, and catch more lines off the superslab, I'd be riding a Wide Glide.
