Agree with Bryan, but most big truck repair shops have an assortment of t-bolt clamps.
Also 1fltri4me too, may need shimmed.
I use a sparing amount of high temp silicone too.

Is the front clamp slid to the front edge of the pipe? If it's not forward enough to where the notches/cut outs areitcannot compress the muffler to the exhaust pipe.
The HD clamps seem to be only good for one or two uses. If they are loosening up you should probably get new ones from HD, or one of the aftermarket manufacturers.
Had the exact same problem after installing SE Slip-ons in December.....I re-used the original clamps, instructions state not too, but dealer said it was fine. Anyway, left pipe kept slipping out. I would tighten and tighten but it would keep slipping out. I bought new HD clamps and installed them....still had same problem. Then I figured it out.....place the clamp near the end of the slip-on like 2kflhrstates. At the end closest to the header pipe you will see a slit....place the clamp there and tighten that baby down real tight. Seems somewhat not logical to place the clamp so close to the end of the pipe, you would think further down the tube would be better, but it is not. near thye end allows the clamp to work I guess. No problems since!!!
All I can say is make sure the V&H clamps are the correct size. I also got the V&H oval slip-ons last year and matched them up to the stock HD ones.
Well, ONE of the V&H clamps was larger, called and V&H sent me a new, correct pair no charge. Went on just fine, no problems.
The torque figure on the slipons is not that high, if you have to crank them down a tremendous amount, they probably don't fit right. The difference between the 2 V&H clamps was very small, but made all the difference in the world in terms of a tight fit. Again, I don't think the torque value was that high in the end. I am too lazy to look, but it's obviously in the HD service manual and probably in the V&H directions.

You'll want to get the clamps over the slits, but not so far forward that slit appears after the clamp band.
