I like them, but wish they were a bit higher, further out.
Is there an extension, or adapter I could buy.??

I am unaware of extensions/adapters like you suggest. but that doesn't mean they are not made...
Most people who are dissatisfied with the fairing mounted mirrors, usually buy plugs to seal the fairings where the fairing mirrors mount. They then remove the fairing mirrors, and just add mirrors of their choice that fit into the clutch and front brake perches on the handlebars.
The mirrors mounted on the handlebar brake/clutch perches are usually higher and often more outboard than the fairing mounted mirrors.
I'm considering that change to my '20 Street Glide.... just not liking the fairing mirrors as much as the perch mounted mirrors on my other bikes.
For the Streetglide mirrors, there are extensions out there that you can move the mirror out, up, or down by a couple of inches. For the handlebar mounted mirrors, Harley makes Long Arm mirrors that you can get.
I'm a big guy, so if I want to see what's behind me when riding my Road King, I have to use a set of Police mirrors. The stems on them is longer then the mirrors that come stock on a Road King. They are a direct swap out with no modifications needed..,,
