Make a pass or two around the axle with black electrical tape until the covers are a nice slip fit and tighten them down. The tape gives the set screw a good bite and centers them on the axle. 1000 miles and still on there tight.
P.S. one of my set screws were longer to fit the smaller diameter axle and they didn't come with shims. Got the P/N right out of the 09 catalog too.

I got the H-D Motor Co set for my bike. Like someone else here said, the inside diameter of the caps were the same. However, there were two sets of set-screw holes in them. One worked well with the nut side of the axle and the other worked well with the round side of the axle. In addition, one of the set screws was a bit longer than the other. The instructions say, clearly, which set screw to use with which side of the axle, at least they did for my covers. They did not, however, say specifically which holes to use.
I had the same problem with the one with the round hole. There was no insert included? Was there supposed to be? I just wrapped two rounds of black electrical tape around the end of the axle, and put it on. Fits great, and the tape is easy to remove for maintainance.
i'm assuming that the tapered ones are the only axle and nut covers that have this issue? wondering about the square ones that are also chrome? perhaps that's a better design. got all my chrome front end stuff and gonna do the lowers and cowbells soon, along with the chrome axle spacers, gotta check the other style caps before springing for them. good heads up on the tapered caps. thanks.
The left side fits fine with the round axel cover. But I am unable to install the right side as the Nut is too small for the cover and the spacer provided does not fit around the nut at all!

The side with the nut shape fit perfect.
Tapered Front Axle Nut Cover 43051-01 : I just had to use the black tape too, in order to get it to fit. Fits fine now looks good. (2007 Street Bob).
