Attached to same shaft = something wrong. Rear must have loose pinch bolt.

Keep an eye on it. Never noticed any vibration, but I've lost 2 in the last yr. lever peg and all just gone mid ride. The second go round I checked the pinch bolt before each ride. Then not 5 miles after leaving work, no rear lever and peg. That time I knew I checked it tight, so the bolt couldn't have fallen out. Lever must be cracking before falling off. Switched to billet stye levers, thinking it might be the cast steel OEM levers are prone to breaking. So far so good, but still checking the bolt every time.

My stealer had to replace mine after it fell off. I was able to find it due to the fact i always shift with the rear shift lever. They ended up replacing the splined shaft and the rear lever under warrenty. Good luck probably just a loose or streched pinch bolt that has now worn down the splined shaft. as long as the bolt is in and the shaft isnt to worn it should not come off.

Good luck,
Steve

The shaft is steel, the lever aluminum. If the lever is allowed to remain loose for a time the aluminum splines will wear allowing the lever to wiggle (as it were) on the shaft. You may be able to cure the looseness by widening the gap in the pinch bolt slot. A hacksaw or cut-off wheel will work for this. If this doesn't do the job replacement is the only remedy. Some use blue loctite (if you re-tighten the bolt you should also re-new the loctite). I use anti-seize and haven't lost a lever since 1995.
