I had that problem also, but I wanted to keep the heel shifter. Didn't have the problem until after I removed the shifters and put them back on to do something with the floorboards. My guess is that the screw stetches a bit a does not tighten as much as before. Anyway, got a new shifter screw at the dealership. It came with some locktight on it. I put a little more on and no problems since.
Mine fell off twice the 2nd time with Red Loctite, put the Chome Cap on it, now don't confuse heel and toe shifting when riding the Sportster anymore ..

Mine was always coming loose after riding. Even after applying lock tite. I called the service counter at the Harley shop and he said it was the shifter shaft. He replaced it at no cost. It took about 20 minutes. Glad it was under warranty.
Any time you mess with those cap screws they need to be lock-tighted back on... I took my heel shifter off shortly after acquiring the bike. Had them on other bikes w/o issue but on the glide it really restricted my use of the floor board.
Mine too came off, after i took it off and adjusted it. I don't believe its a locktite isue or a screw issue really, its just hard to get it tightened enough to grab the shaft again. I had to use an equilizer (jack handle) on the end of my ratchet to get enough leverage to tighten it enough. Since then, no problem. Hated being without it.
Mine fell off a month ago. I was able to retrieve the shifter in the middle of an intersection. Notified the dealer and he replaced the shaft and shifter under warranty. Knock on wood, so far so good.
i too remove mine and put the cap on. in the past i have used the spacer and moved the front shaft to the outside position. but i like having the cap on the outside position better. i would rather lose it than the shifter.
the newer model levers have a larger bolt in them that can be torqued much better. they seem to stay tight with the larger bolt in them.
my rear shifter stays in the saddlebag for emergencies.
