Maybe it's just me but I'm not overly concerned with immediate notifications of my loved ones if I go down. I do keep my phone on me with ICE #s but that's the least of my worries.
My priority is getting proper and immediate medical care. I just want to survive and can track down family when I get around to it (they will come looking if I'm missing). Bottom line is that friends and family may comfort you in the hospital, but they aren't going to make a difference in your survival chances during the first 24 hours.

I had an app called cradar when I had my droid. It senses a hard shock and you have a certain amount of time to deactivate or it automatically sends your coordinance to an emergency contact. Not available for iPhone yet though.

Life is full of worries...If I worried about every little thing I wouldn't leave the house...I figure if it's meant to happen there is nothing I'm going to do to stop it so I live my life as though it was my last everyday. Some people just worry too damn much! Sometimes it's nice to ride alone, if something happens I'm sure my family will find out.
If everytime I went out on my own I left a route, contact #, etc, etc.... Well, I figure I may as well stay home where everyone can see me.. If it's my time then it's my time - that's the way I look at it. I apologize if people disagree but that's how I feel. My ride gives me independance and a free feeling, if I don't have that then I wouldn't want it.
I understand and feel for you. I ride alone as you do, 99% of the time and I don't think anyone really gives a damn where I am. So I just go but I always take my phone. The heck of it is I often head to the mountains of upstate NY where there is no cell phone coverage.
