Default this fell off while riding
I knew something fell off while my son and I were riding but couldn't find what it was. That was 2 weeks ago. Glad the wheel stayed on.

Yikes. I once had the top clamp from my handlebar riser fly off while riding. Luckily there were two of them but the feeling was frightening. You're very lucky.

Damned lucky I'd say. Looks like your rotor might have a crack too.
I'd have a REAL close look at everything, then I would kick whomevers **** that did the last install. If it was you, well then, please be more diligant in the future!!
