I am with the crowd, I have always started my bikes in neutral and the clutch pulled in, its habbit. Dunno if it will start with the clutch out in neutral or not never tried.
I seldom pull the clutch in while starting unless the bike is in gear. Since I am aware of whether the bike is in gear or not since I am generally rolling the bike out of the garage before starting and seldom leave the bike in gear when parked, I have never have seen the reason to do so though it is always one of the first steps taught in motorcycle safety courses.
Makes absolutely no difference on newer bikes with a nuetral safety switch as they will not start in gear unless the clutch is pulled in. Some earlier models such as my '02 will start in gear, clutch in or out.

I do the same. It's rare that I leave the bike parked in gear. I remember parking on a slope and left the bike in gear. Returned to the bike and burped the starter while in gear.
