I have had chronic back pain for a long time, and a whole lot of Birthday's has not helped my back. A mustang solo with a back rest on the Road King..Also a Kidney belt that is pretty wide.I would suggest you try a wide back brace that keeps your posture from slouching.. Good Luck with that..
Many riders complain about the stock RK "Buffalo Bars" positioning. Try the Police RK bars. Makes the difference for long days of riding. HD must have taken note of that, due to the fact that from 2014 models and on, all RKs now come with the Police style apes.
My 2012 Road King gave me a fit in the back and neck till I got rid of the stock bars. I went with the road king high bars. Just an inch or two up and a small pullback. Worked great for me. If I were to do it over I'd probably just get the riser setup Harley sells for the bike and raise it up and inch or two that way. No dicking around with cables etc.
Some good advice here. You will have to find the set up that's best for you but good posture when riding helps a lot and a back rest will help with that. I start getting a pain under my ribs on my right side if I let myself start slouching too much.
I was born with scoliosis and have some bulging discs from that as well as degenerative disc disease. Doing my best to avoid going under the knife. For me it was a new seat with a more adjustable back rest that sat me back a bit (I'm 6'3"), and new reduced reach bars to let me lean back into it.
Stock seats suck, especially if you are a big guy.
Heels on the boards, toes on the bars for me too. I can go about 300 miles with just stops for gas and water in/out on the Ultra without too much pain. Take a little break and do a few hundred more.
My back pain was a lot worse until I started getting out and walking and cut the sugar out of my diet. Now a couple Aleve does me pretty well where it used to be hydrocodone every day.
1.5 year from now my back started to kill me. then I traded for the Fat-Bob and it become nightmar. After thet my knees start... Doctors, cortizon ..... then 1 day I understood that I can put some $$$ on seat, handelbars, ...,..., but the real truth is I have to take care of my self as I'm geting pld. At the age 53 I start to do Yoga, lost 23 paund and now I dont have knees problem any more, the back is much better and on my RKC I have stock everything.
I do need to moove to haritage handelbars but most of it is the way I'm holding my body in riding time.
