I don't know the model number but there are 2 stickers that say wild1 chubbys and one that says works with hydraulic clutch (it was standard cable on my bike). It measures right about 9" of rise. I pulled off and went to 16s. Used condition with some scratches and scuffs but still looked great to me. Shoot me an offer. I think ground shipping is around $35.
