Fuel issue
My girlfriend has ax 2003 Dyna Wide, we left the other day and about 5 miles it died like it ran out of gas, stopped restarted then it ran about 30 mikes and did the same thing. It did the sane thing a least 10 times. Sometimes when she would restart it would take a while lijevit had no gas.

Could the petcock be bad be bad or the petcock filter be clogged??

Yes, to both of those questions
The petcock has a vacuum actuated diaphragm and there is a filter screen, after 14+ years the diaphragm may have failed, you could have a vacuum leak in the hose running from the carb that is preventing the diaphragm from opening, the filter screen could be covered in debris, or the screen itself could have disintegrated and is clogging the petcock.
Also, there's a potential for carb related issues as well being the cause, but the petcock would be my first stop
If you do find the petcock is the culprit pick up a decent gravity fed petcock like a Pingel Power Flo; it isn't vacuum actuated, the vacuum barb on the carb is capped off with a gravity fed so get in to the habit of always turning your fuel off after every ride with a gravity fed petcock

The carb was rebuilt a couple of years ago.
She he did run the bike almost out of gas several weeks ago.
Then the next time we rode the issue started. When it is running it runs good then all of a sudden
it acts like out of gas.

We'll assume for now the carb isn't the issue with a recent going through.
A few years ago I picked up an '03 Wide Glide 100th Anniversary numbered paint edition for a great price; the previous owner wasn't too mechanical and had all sorts of issues keeping it running, etc.
One of the very first things I did was pull out the petcock, the level of nasty and crud that came out of the tank was amazing. The petcock screen was torn letting crud make it to the carb. Pulled the tank and rinsed it out, cleaned and rejetted the carb, installed a Pingel petcock, and rode that bike the rest of the season trouble free; she was completely restored over that winter. Never should have sold that bike
It's very possible by running the tank quite low if there is debris in the tank the screen could have picked it up and it's now blocked. Pulling the petcock is just messy, but not difficult to do. You'll know if there's debris and crud in the tank and the condition of the screen. If it is torn and there is debris in the tank it could have made it to the carb.

Her bike is the Anniversary model as well.
I just order a new Petcock as you had suggested.
Going to drain the tank and remove the petcock today
and inspect the tank and filter.

The symptoms sound exactly like what my 1998 Wide Glide was doing. My bad luck was as I was on my way to OC bike week. I limped it to the local dealer and had them pull the petcock as I also thought it was a gas flow problem. They showed me the filter and it was covered with my gas tanks inner liner which is peeling off. They cleaned it up as good as they could and I put over 1500 miles on it so far with no more problems but I’m looking into resealing my gas tank for a winter project.

Pingle "you gotta turn it off" petcocks go for $100+ and the OEM vacuum "auto shutoff" petcock that worked well for you for 14+ years goes for around $75...
I had a lot of peeling liner on my 2001 FXD that I could see floating around but it never interfered with fuel flow. My OEM petcock developed a leak in the diaphragm that drove me nuts tuning my carb until I discovered it. I replaced it with a manual actuated petcock that worked great until I dropped it on the right side backing up in a parking lot and dumped fuel all over the place before I could get it turned off and the bike back up...
When I painted my tank this summer, I cleaned out all of the liner and put in a $78 new OEM petcock from Newcastle...problems solved.

The easiest to check is the fuel cap vent. Gas won't flow from the take with a vent plugged up. Next time it dogs out loosen/remove the cap. My brothers semi tractor wouldn't flow enough fuel and the turbo wouldn't spool up, found gum on the fuel caps from last fuel stop.
