Yet another pulley bolt thread

I don't have anything to add to the OP problem but looking at this, how does this happen? Looking at the photo it doesn't appear that the bolts just backed out as we assume could happen if someone didn't use a locking system. This is also not the first post like this. I can think of a couple of ways and one is just plain scary.
First scenario and the one I would be less worried about if it happened is the backing plate for the locking system is bringing the bolt head out just a bit too close to the swingarm and with some flex in system combined with closer tolerances that bolt head is coming into contact with the swingarm and things cascade from there.
Second and more worrisome for me is somehow the bolts are jumping threads. Maybe these bolts are like connecting rod bolts. 1 and done. After they are torqued down once they stretch too much to be used a second time. Maybe harley has some weird thread cut. Maybe both. Maybe something else.
I guess as we have discussed before replace the bolts with new once removed.
