PM vintage 4-piston is the route I went. It bolts right up with the included adapter. On my bike it was centered on the disc, too, so I didn't even have to shim it. It was around 400 bucks. I also changed over to a braided-steel line which was another 60 or so. Much better braking than the single-piston that was OE.
Yes that for a 40mm (mounting hole distance) Caliper. They are a bit less plentiful it seems compared to the 65mm.

I just search for "Brembo Caliper" on ebay and visually pick them out
There was a post by forum member dfwhockey17 (right in your neighborhood ), but the photo is gone now. It showed exactly what were talking about 40mm Brembo with adapter on a FLST...you might reach out to him (seems to be a "softail guy")

Its funny I actually think I know who that is. If its who I am thinking of, we couldn't come up with any actual part number for the caliper, just have to search like you suggested. and I am having a hard time finding them.
