The handlebar position is a personal one. What worked on my Sporty didn't work on my Softail. Have the ISO with throttle grips and those are great, although they do tend to put your hand in a oh so slight eliptical environment vs. round. Only thing I can say is that you don't want your wrist bent up or down because in those positions that is what compresses the nerves that causes cp. It took a good 100 miles of riding and adjusting to get the bars just right for me.
I also found thta the thickness of gloves also affected my position - thicker gloves like in the cold weather riding has me holding my wrist differently than when I wear the thin perforated summer gloves.
But if your condition is that bad, I would highly suggest the surgery. Today's procedures are far different than 10 years ago and the recovery is like 2-3 weeks. You can use your hand for limited use like in 3 days after surgery. And gosh they feel much better.
