Red thread locker and torque to 60 ft lbs. We always install the pulleys on the wheel on the tire machine and it makes it much easier to get the proper torque on the bolts.

This. Plus I aways eye my rotor, pulley, axle bolts etc every so often. To many bad stories of these things backing out. Glad you are okay.
And if he makes it right put a torque wrench on it when you get home to make sure. I had loose lugs from Discount tire one time on our family car. Always check. You never know.

Sorry about your problem, but loctite, or lack thereof, had nothing to do with it! Neither did a torque wrench. The bolts wer never tightened - simple as that. It was a mistake on the mechanic's part.
I gotta laugh at all these posts suggesting you have to tell the guy how to do the job, what materials to use, to check the torque when you get home, etc. If that's the case, and you think you know more than you think he knows, then why is he working on your bike?
Stories like this make me feel for you guys that have your bikes (or anything) worked on by others.

One thing I hate is when a customer wants to look over your shoulder while you work. I usually stop and strike up a conversation until they realize that talking cost 85 an hour also.
I took the day off and stopped by the shop this morning. He said he had no idea what happened...said he used loctite and put an impact on them. I could see where about 1/2" of the bolt was darker, but shouldn't there still have been some visibly red locker on there?
He told me he stands by his work and will fix/replace what's necessary. Had one of his guys pick up the bike and he didn't know either...told me they use red loctite on pulleys and has never seen this happen before. I don't know WTF happened, but they have to be mixed up somewhere. The damn things backed out one by one and I double checked this morning...only 100 miles.

Ha, sorry I didn't take any pics. Probably should have. Injury pics would make me sad later anyway.

Thanks for the specs. How much torque do you put on your rotors? I will double check everything when I get the bike back...guess I should have the first time around. You've got an order for another Shelby pulley coming soon because the first one's banged up and a little warped.
What happened was they never tightened the bolts. Plan and simple, someone got distracted or whatever. In my book, there's no way, even without loctite would the bolts have backed out. Just isn't going to happen.
At least they're going to make things right and I hope that includes fixing the cosmetic damage to the swingarm by having it PCd and not just some touch up paint.
Ouch...good thing you caught it. Know ya one peesed off dood but go easy on him. One thing I've found is going off on the mechanic might be justified but...
I had my pulley bolts come loose about 6 months after installing a set of wheels. Check them regularly and red loc tite and torque to 60 ft lbs. That blows.
