Softail slim forward controls
Jaybrake, Accutronix, Battastinis, Performance Machine are four companies that make good looking forwards (according to the pics I was looking at) - can't say specifically what fits what, as I didn't get that far down the line. I'm sure there are other makers, too... those just came right to mind.

I used CycleVision's Go-Forward kit, and a "custom" brake line that I put together from premade parts. It ran me about $200 for the whole deal. Here's a link to the thread I posted about the project: https://www.hdforums.com/forum/softa...lim-bingo.html
On the Slim, this also moves the side stand forward a couple of inches, which alters the way the bike rests on when parked. It's still plenty stable, but it leans further than it did originally.
Looks like a couple of other options are also out there, though, too, depending on your bike year/model. Joker Machine has a 3-inch kit that includes a brake line. CycleVisions and Accutronix have a 3-inch kit that you need a separate brake line for. Arlen Ness has a nice 3-inch kit that includes a brake line (braided stainless, I think) that lists at $200. I wanted a black brake line, so I didn't use the AN kit. I found all of these options in the Drag Specialties catalog - sure you could do a little looking to get best pricing.
In order to install any of these kits, throw the bike up on a lift, pull off the shift linkage rod, the 3 bolts holding the control mounting plates on the frame, and the brake line. Bolt the extensions back on, bolt the controls back on, hook up and bleed the brake line, and hook up the shift linkage with the new rod. Good to go. I had to build up the rubber bumper on the side stand, 'cause it didn't touch the cover anymore - the ball on the extension did - and it would rub a spot on the cover as well as be nearly impossible to deploy using my foot.
