There are some small sleeves and bushings in there that have a habit of springing for freedom when you take it apart. Remove it and print off the appropriate exploded view from somewhere like ronnieshd and compare to see if your missing something.

The cables have adjusters in them, to take out that slack, assuming everything else is OK. Have you done any work on your bike recently?

They're supposed to pop out on the 95+ models. The little clip in the cable can be designed a little off tolerance (if you're using bad aftermarket cables or worn ones) and they're looser than they should be. If the ENTIRE cable pulls out, it's not attached to the throttle sleeve inside the switch. They should be tight enough you can't really pop them out when properly installed and everything is working.

Where the cables push into the housing they have a circlip for want of a better word. That clip springs out to hold the cables in place.

I have put new HD cables on a bike and now I cannot get the cables off. Either those clips are damaged, lost their spring or are not present at all.

To remove a cable does just require a downward pull. How hard are you pulling on the cables?

Of course you can take the slack out of the cable with the adjuster to stop the cable falling out but to just take all the slack out for it not to happen is not the way the cables are supposed to be adjusted

You are slackening the lock nut off first?
