This is the riser on a Custom. It is clear the extensions can only go on one way, I am not sure where you think there will be a gap?
The only thing the OP needs to decide is with extensions the bars will also be moved closer to the rider but with the angle of the yokes it would probably happen if the extension was put on first.

Indeed there would be a gap but that is for the OP to decide. I didn't link to them but just saying they will fit. It is how risers work though on any other bike. On my stock bike there is a small gap between the bars and top yoke.
Here is what the extensions look like when on a different bike.
I agree with putting the riser below as pictured. I originally posed the question about longer bolts but as someone has done it it can be made to work.
