My trouble always comes from the shaft (part 15) spinning within part 16. I've tried tightening on the road, taking it apart and today rotating the shaft to get to new splines on the road and it still fails. It just seems like no matter how tight I try to get it, once worn there's no hope. Actually I just had it replaced last week due to this failure AGAIN.

McMaster Carr is your friend. I got my Heim joints from there. they have 3" thick catalog full of good/cool stuff. go on line and check things out. Fasteners? they have them. misc steel/alum bars/tube/rods...they have it. all sorts of shop stuff. they ship quick and I use the hell out of them for work.

Same here. 15 and 16 is a problem all the time for me. Someone told me to replace the bolt because it's stretched but I haven't tried it yet.

so what's the problem with it failing on an interstate? you're still in gear and can make it to the next exit so you don't have to stop on the side of the highway...

Default Shift linkage

As many have said, heim joints fixes the problem but also make sure the shaft that the shifter mounts on that goes through the primary case is tight/secure and not able to vibrate loosely , not only does that help to destroy the rod joints it will help to strip the spines on the shaft coming out of the trans with the shift arm on it. Slide the shifters on as far as you can and squeeze the rubber bushing tightly before you tighten them in place and now your linkage won't vibrate.

Excellent price for a heim joint linkage. If you use one, how's the quality?

Yeah, I know. But the "pucker factor" would have been a bit bigger at 80 mph than at 40.

The Heims will fix the rod end failures but if the nut backed off the bolt not sure how that helps?

Seems to me that the bolt and nuts should have cotter pins in them.
