I’ve used them on the skid plates of snowmobiles. Never had an issue tightening them.
I have had one not thread onto a bolt before. Have you checked to see if the bolt screws in without hanging up on the brass nut?

sounds like you got something buggering up the threads or you got it going in cross threaded. you should be able to screw the screw in all the way by hand and then use a screwdriver to tighten it up
Try screwing the screw in without putting the rubber part in the well hole as stated above to make sure the screw goes into the nut. Then put a few washers between the screw and the flange of the well nut and tighten the screw so the nut pulls into the rubber part and expands the well-nut some. Then install it into the hole and it should be a press fit. so it holds the rubber part while you are tightening the screw.
Had the same issue with my Honda VFR Fairing well nuts as some of the holes in the ABS fairing were a little larger. Another trick is wrap some electrical tape around the well nut to make it a tight fit in the hole so it does not turn.

When I replaced my stock headlight with LED on my '20 RK there was one of these inserts spinning. It pulled out of the nacelle when I removed the ring.
It appeared to me that these were supposed to act like a rivet when first installed. So I re-inserted and held the back half with pliers while I tightened the screw. That worked for me. The insert is meow tight like the other 7.
