Simple answers, DOT, lawyers, operator brain and muscle training. Under stress, most people lose creative thought and revert back to habits and learned behaviors. Stick around long enough and you will see a "war story" about a cruise control malfunction or a stuck throttle. Under stress most people can't instinctively remember and try all of the ways to shut off a malfunctioning cruise control, they also won't think of the kill switch practically under their thumb. They either eventually force one hand off the handle bars and turn off the ignition or take a death grip on the brakes and hope they stop before the brakes over heat to uselessness.
There are occasional stories about runaway cars, whether it is caused by a jammed floor mat or a true malfunction, the result is the same. The victims don't think to shift to neutral or turn off the ignition. Every car I have ever driven with a steering column ignition switch had an OFF position for the key switch, and then one or two more clicks before the steering wheel would lock. How many of you now have cars with electric power steering? Can you steer in the event of an electrical failure while rolling? Have you tried it? How much effort is required?
