Toe only, never used the heal shifter, couldn't get used to it after 35 years without.

I use both. A trick I learned was to adjust both shifters down as close as they would go to the floorboard and allow shifting. I don't like lifting my entire leg to engage the heel shift, so adjusting it low allows me to just lift my heel and shift. I don't know why they come from the factory adjusted way up there requiring you to move your foot a long way to shift either toe or heel.

Default heel/toe

I use toe only. Removed the heel part and covered the splined shaft with a cap from Harley.I have been shifting up and down with my toe for too long to change now!

Toe only.
If you've been riding as long as I have you'd say the heel shifter just isn't natural plus it takes away from floorboard space.

I remember getting used to my forwards with my softy and almost breaking my ankle when I brought my feet straight down for the center pegs that weren't there. Once I got used to the forwards I wanted nothing else. Same goes for the heel/toe I have adjusted the heel as low as it can be to up shift and love it. I always thought it was a great idea and tried to find a lever for my softail too. I won't be changing it any time soon!

I always use the heel shifter. If I remember right, it took a little while to get used to when I first bought my bike, but now I can't imagine being without it. Besides no more scuffed toes on my boots.
