The locking tab on mine never locks. Is there a way to adjust it?
There isn't any adjustment as such. If "tab A" isn't going into "slot B", then there is something not mounted correctly, or something is broken. I've seen the tabs get worn over many years, but that was very rare.

At the P.D. where I work this past July 4th. One of our motor guys was taking a left at a intersection at fairly slow pace when the kickstand malfuntioned and went down. The kickstand stuck on the pavement and jack-knifed the bike. He broke both of his wrists and bruised his ankle pretty bad. He was riding a 2006 Harley.
How the heck would that happen? If the kickstand went down when he was moving I would imagine it would just drag, not lock. He must have forgotten to put it up before he took off.

Its not that unusual for the kickstand spring to come off. When I was in motor school we had kickstand springs coming off all the time. Motor units see a lot of unimproved terrain that cause the springs to come off. I would be careful around speed bumps. I had the spring come off on a motor unit while riding on patrol when I went over a speed bump. After the spring comes off the first time it seems to come off easier. I had to lay the bike over to get the spring back on. I carried a leatherman to get the spring back on.
