I tried to get used to it because so many folks swore by them but took it off after a few weeks. Just not enough room to move my foot around. Kept shifting when I didn't want to.
I'm surprised that HD dosen't ship the bikes without them then make those who do want them pay some outlandish price.
Removed mine a couple of months after I bought my bike.

Update:
I bought an extended heel shift this past week and installed it. It is a great fit. Extends the shifter by 2 1/4 inch. Sets the shift peg just rear of the foot-board. I like a heel shift, i just don't like the crowding. This took care of that problem. Extended shift lever is made by Walter's Workshop.
I didn't like the HD solution, and I was too impatient to wait and get the other aftermarket one, so I made my own solution...from a steel spacer I bought at Ace Hardware for a couple of bucks. Total cost including the chroming was about $20. Works excellently so far...
