With the clearance between the washer and axle shaft flats, sometimes that washer will move when tightened.
There was thread about this on the V-Twin forum recently.

Your axle cam pic is all wrong. Go to a dealer, bend over, look for yourself!
Removing/replacing wheel, adjusting axle cam & belt requires common sense. Clue in before ya blow another belt!
What?.....................That would completely negate the function of the entire mechanism. And in fact it would guarantee that the axle would be mis-aligned. The whole purpose of the cam washer, on the axle, keyed to the axle is to guarantee that when the cam washers (washers as in both of them) are up against the nubs on the swingarm frame (that's nubs as in both of them)..........then...............your wheel is in alignment.
I was refering to the post that said he had a lot of slop in his cam washer.
If he has that much, he could make a shim that would take some of it out.
If you make a shim to go in the back rounded part of the cam washer to pull the flat of the washer to the flat of the axle, it won't change a thing except make sure your washer stay's where it's suppose to.
Does anybody on this forum measure from the center of the axle to the center of the swingarm shaft? That would assure a perfect alignment after belt adjustment.
Trackadapter, Look on Youtube! Their's video's on belt adjustment! One guy made a measuring tool out of a piece of welding rod with an o-ring that slides on it. Geeeesh, we're not talking rocket science here!

The service manual shows the adjusters up against the block. Also says if they move to start all over.
I don't see what buying a axle/nut and cam is going to prove other than that their still idiots.

I'll go out and take a picture for you. In the meantime look at the edge's of the broken belt. If it was that far out, you will have rubbing and fraying on the edge of the belt.
