at this point the teeth on the shifter and the shaft may be worn.

try swapping the front and rear shifters and tighten

otherwise alot of guys remove the heel shifter and they can be bought for cheap

Red locktite? You could also safety wire it.

Do you use it? If not, then just remove it and get the little cap they have to cover the exposed shaft. If you do use it, I'd try replacing the set screw first. If that doesn't do it, then try switching it with the toe shifter. If that doesn't do it, then buy a replacement. BTW, make sure that the set screw is seating in the little grove in the shaft.

Is the problem the set screw keeps loosening or that the teeth are wearing?

Go get a new shaft, and possibly a new shifter. The splines are evidently worn.

My heel/toe shifter AND spline fell off the bike on a roadtrip. Never to be seen again. The lever arm on the back side still had the bolt in and was tight. So yeah, the splines wear.

They do..Once the shifter is clamped all the way together, all you do by tightening is stretch the bolt.

I believe the OP is asking about the shifter peg and not the shaft. I lost my rear Willie G shifter peg (found every piece of it too) while riding one morning. Went to put it back on and realized I had installed it wrong to start with. I had it installed so that skulls faced each other not noticing that they only mount 1 way. Put it back on the way it was meant to be and haven't had any problems since. Hope this helps.

Get rid of it. It's a much cleaner look without the heel shifter and there's more room for my big foot. Just my opinion.
