I have been using Bates Tactile sports boots very comfortable on the bike and also off the bike no more changing the boots to take a walk after done riding.
Dr Scholls Custom orthotics work well with any boots. These are the ones you stand on the machine and it gets the hot spots on your feet. I got mine at Walmart, I think they were $50, but they have a money back guaranty. I have flat feet, and most boot/shoes have to high of an arch for me. My pain points are my heels. These things work well, but do require a little more room in a boot. I bring them with me when I buy shoes, and get the appropriate size for using them. (I typically need 1/2 size bigger then normal, but not all the time)
That being said, I have found a combo of Bates 8" gore tex boots (laces and zipper on the side) with these insoles are a perfect match for me.
