I have a 14 xl1200t, risers are cast into the triple tree. I looked at changing the risers or bars, there were some spacer blocks that would lift them straight up, I looked at a lot of different bars. If you go the bar route, keep an eye on the width tip to tip, she might not like them much wider than stock. I never pulled the trigger on bars though. It's an excellent bike though, totally enjoy it, just painful leaning forward all the time to reach the bars.

we were going to swap the upper tree but way too much work.

Then started looking into the Buckhorns as they have a rise & pull back.
This all stated after she swapped out the seat so I am thinking the extensions and lowering the bars maybe the ticket??

Might catch some flak for this one, but while I was looking at all the options, I found Rox pivoting risers, seems mostly metrics are comfortable with this, but what I was thinking was a set of those would let you play around with the positioning of the hand controls. When you find where it suits her, take the dimensions from the risers up to the hand controls and then you'd know what dimensions on new bars to get. Or just leave them, heck with what people think, if it makes the ride more enjoyable, it's much better having a bike going down the road than sitting in the garage because its no fun to ride. They came is 1 to 4 or 6" lengths.
I'm 5'4" on the same bike with a Mustang seat, I can vouche for your wife on reaching for the bars being a complete pain! Just moving to a softail in the next week though, but of course have the same handlebar issues with the Heritage...
