I hadn't tried the anti-seize thing, and still put LT blue on the threads as well. One thing that can happen is that the lever opening (internal splines) can stretch and you can tighten until you snap the bolt off, it won't get any tighter because the ends are together and you are essentially trying to compress the metal. A small file or running a hacksaw blase through that opening when the bolt is out (off the bike obviously) will open that space a bit so that you can get more clamping force out of it. Essentially though, once it's been stretched to that point, you're only buying time. I have found that if you put LT on the splines as well that once it's set, it eliminates the slight wiggle that's left and will keep it from trying to walk off of the shaft. I had to reset mine every few rides, like monthly, haven't touched it since putting locktite on the splines.

The shifter lever won't fall off the spline shaft because the set bolt that pinches the lever together aligns with a sort of slot in the spline shaft. Now if the bolt fell out then yes, the shifter could fall off too.
Yeah, filling the gap wider will buy you more pinch space for the lever. I did that just before a bought a JayBrake set for my 06RG. Eventually my JayBrake kept coming loose too, so I drilled it completely through and used a longer bolt with nut until I got a replacement shifter.

Thanks for all the great advice and tips. Update..... I've been riding it all week now with just the heel shifter. It's been hot as hell here all week and by the time I get home our driveway and garage are just way too hot to mess with it. I'll post back when I finally get around to fixing it.
