Lost mine on my Ultra last week as well. Take a look at all of yours.The bolt that is suppose to pass through the groove on the shifter shaft doesn't touch the groove at all. The only thing that holds it on is the tightness of the allen bolt. I called the service writer and he gave me a new one under warranty. You might want to give your dealer a call.

It should be covered under warranty,why pay for one when your dealer owes you one for not checking it or making sure it was tight during setup or the 1000 miles service.
