Had one on my 2013 Ultra Limited when I hit a deer last month.

So, if I didn't have the bell I might not be riding at all. Who knows?
Have you ever seen a Gremlin (not AMC) while riding with a bell? OK then.
Follow the rules.
Can't buy your own, has to come from someone who cares about you.
Mount as close to the road as possible, as Gremlins are hard of hearing.
Check it when you wash to make sure it rings, if you can't hear it, they can't.
Yes all mine have bells and when I sell one the Bell stays with me. I have one that I know is 15+ years old.
I knew most of what you posted, but didn't realize it was supposed to be close to the ground. I have mine hanging from the clutch cable near my handlebars.
Just recently got my bike back from the shop after not making a turn in the north Georgia mountains. $7500 worth of damage to the bike and a small scratch on my pinky finger is all I suffered.
I like the lore, as many have mentioned, but agree with one of the first posters...I am spiritual, not superstitious. The good friend who gave me the bell is a dear friend and the act of giving it was what it was all about.
