Default Lost front brake pad

The 1987 FLHC is running perfectly until yesterday when a metallic sound could be heard at low speed. I shut off the engine and it was coming from the front brake calipers. A close look showed that the outside puck on the right side of wheel was missing. I heard it fly off earlier but thought a piece of wood got picked up between the fender and the tire and was lodged there. The rotors are new floating billet chrome and the brake pads are brand new 250 miles on them. I've never seen this happen in my million mile history. Anyone have any ideas why this happened? No warpage (smooth brake engagement) New pucks going on this morning.

Someone didn't know what they were doing and did a sloppy rebuild.

The fiber part of the pad. They're called pucks on BMW's. Term stuck with me. I take this bike to a retired tech so I figured it was in good hands.

Never heard of the fiber leaving the metal backing of a pad. Never!

If just the fiber pad is gone, then a bad pad, if the entire pad came off (backing and all, see below)

Check out link below, even though for a sportster, it’s the same components.

Especially steps 13, 14, 15.

Step 13 shows the proper install of the alignment peg, after you clean and lube it.

Note the red square box showing you the locating rivet in step 14.

Step 15 shows proper positioning of alignment peg and rivet. This is one of the most common mistakes-not getting it positioned correctly. Hope that link helps you out to at least double check your work.

Perhaps the bonding agent was not put on correctly at the factory. What type pads (pucks) were you using?

Thank you for the clarification. I have an old Evo.. ‘85.. bought it new and have never heard the front brake pads referred to as “ pucks”.
