I have the cable-operated H-D backrest. Works well, helps posture and allows you to move it forward/back while on the road to adjust position. Two things to consider: #1 make sure there is room for your passenger with the rest installed (or remove it and use her as the backrest), #2 The unit does not fold forward very far (for ease of passenger access), if you push it too far forward, it will tear your seat apart.
My wife can't ride with it on at all. And when i first played with it, I pushed it forward thinkin it was pretty cool, and then saw how it was pulling on the seat threads and never pushed on it much again. But it is one of the best as far as adjusting goes.... not sure how many others can be different heights, move front and back, and do it with such ease like HD's. But it also only fits with certain seats because of the LARGE mechanism to do all that.
I have the Utopia backrest... the best you can get Can be adjusted forward...backwards & up and down... no cables required... no bracket need to bolt to your fender. It is somewhat wide but moan... it feels so good to have all that support on long rides!
My Utopia sits much lower,closer to the seat, my thin stock 09 RG CVO seat looks lousy with the taller HD one that's made for thick stuffed seats ... also Utopia has a 1/2" set back to push it further back if you need it, as I like to sit way back ... & has optional quick release pull out, no tools needed ...
mine mounts / bolts to the fender just like the HD one, but they are not interchangeable ...

My only knock against the so called "non-adjustable" HD backrest is my arthritic hands make it difficult to install and take off. Just don't have the grip I used to have.
Love my HD "non-adjustable". Works great, and is a simple robust design. No need to take it on and off. Adjust it the way you like when you install it and just ride.

That's the reason I won't buy another one to use on my road glide. The one I had on my road king tore up the seat real bad, you just can't help pushing it too far forward. Otherwise it's a good back rest.
